Commission Order Form

Since my new prices go into effect today, so does the new process in which ordering commissions. I've had a few people already test a prototype of this form, well, now it's time for the prototype to be released. ALL COMMISSIONS, starting today (Dec. 1st, 2014), all commissioners must fill out this form, and submit it (either by mailing a copy to 3228 Military Road, Malvern AR, 72104, or by scanning it and E-Mailing it to my GMail address: Tank50us@Gmail.com) BEFORE I start your piece. Funds must also be in my hands (well, paypal account), before I start. If for what ever reason you feel you can't afford the price up front, CONTACT ME ABOUT IT. I'm willing to work something out so that we both win out
